Sample tattoo consent form clause: client consent

I confirm that I am 18 or over and have shown photo ID. I have read and understood this form, and the information I have given about my health is true and complete. I understand that a tattoo is permanent, that removal is difficult, costly and may not be complete, and that the final look can vary with my skin and how well I follow the aftercare. I understand the risks, which include pain, bleeding, swelling, infection, allergic reaction and scarring. I have approved the design, spelling and placement. I have been given written aftercare instructions and I agree to follow them.

Questions, answered.

Is there a tattoo consent form sample I can read?

Yes. The consent clause above is copied from the Tattoo Consent and Medical History Form in the pack. The rest of the form covers ID, medical history, the design and the inks used.

What should a tattoo consent form include?

The client's details and date of birth, the photo ID you checked, a medical history, the design and placement they approved, the risks, the inks and batch numbers used, their agreement to follow aftercare, and explicit consent for you to keep their health information.

Can I tattoo someone under 18 with their parent's consent?

No. The Tattooing of Minors Act 1969 makes it an offence in England, Wales and Scotland to tattoo anyone under 18, and a parent's permission does not change that. The only exception is a tattoo done for medical reasons by a qualified doctor or someone working under their direction.

Do tattoo artists need a licence in the UK?

It depends where you work. In England outside London, the artist and the premises register with the council under the Local Government (Miscellaneous Provisions) Act 1982, and the certificate and byelaws must be displayed. London boroughs usually license tattooing under the London Local Authorities Act 1991. Wales has required a special procedure licence and an approved premises certificate since 29 November 2024. Scotland requires a skin piercing and tattooing licence. Check with your council.

How long should a tattoo studio keep consent forms?

UK GDPR sets no fixed period: you must not keep personal information longer than you need it. Studios licensed in Wales must keep client records for 3 years. The privacy notice in the pack suggests 3 years after the last appointment, which you can change.

Can a tattoo deposit be non-refundable?

Government guidance on unfair terms says deposits you keep should reflect what your business actually loses, and a deposit kept whatever the reason for cancelling is unlikely to be fair. Clients who book online or by phone also have 14 days to cancel. The deposit policy in the pack is written around both.
